trying to find out what causes a FPR to die, as not sure if its the problem or not, only think i can think of is that it was run dry for a short period, or does it need liquid to operate ?

Its not a powered unit, therefore is only working if there's fluid there. Running an FPR dry is just like running a fuel line dry :)

Its either just old and fooked, or it was connected up the wrong way.

no, its just a relief valve, theres a diaphragm and a needle valve in it with a spring the force/pressure relationship along with the offset of the vacuum on the spring side of the diaphram controls the lift of the valve and therefor the backpressure of the fuel rail.
