"skimmed" means to ahve the cylinder head taken to a machine shop (engineer) and then have either a small ammount taken off the combustion chamber side of the head to remove any minor deformaties eg warped from overheating OR you can have lots taken off to increase the compression ratio to compensate for cams with long inlet opening times or just to try and squeeze an extra horsepower from it.

"Ported" means to work on the inlet and exhaust ports of the head by removing material from key areas to increase airflow. You have things done like removing the rough casting marks left by production and restricitve parts to make the engine breathe easier, thus making more power as its got to do less work to get the air into it.

both thigns on their own give neglegable gains unless extremes are gone to, but along with other mods such as improved exhaust manifold, better intake system and an uprated cam youcan see really beneficial gains.
