Rear Disc Conversion

Hey all.
I know theres a guide to this. I have this but what I would like to know (or have a list of) are the exact parts and what they come from to do this.

the original axle stubbs have to be cut off and welded onto the nova ones, and holes redrilled, easy!

the stub axles unbolt. you cut off the backing plate, and replace it with that from the cav/astra axle, or make your own. (i'm sure you can buy them but can't see the point myself)
