If the beast passes tomorrow i will need to tax it, it is declared sorn and i dont have a renewal form. What do i need to get tax from the P.O?

v5, mot and proof of ins? (cant remember if you have to fill a form in at the po)

As said mot, v5 and insurance certificate. no form to fill in just give them all of these docs and say 6 months or 12.

Wrong, if you don't have a reminder (V10), you should fill in a tax application (V11, of which the post office will have. They may even fill in the details for you). You'll need the usual MOT & Insurance cert, and also the V5c.

i took my v5 down with my new mot cert and insurance cert and all they done was to click the barcode on the v5 and said 6/12 months

Yep, V5 and the normal docs is all you need.
